Usually multiple providers, several conditions, a complex treatment pathway or surgery, or a lot of family coordination. If it feels complicated to you, it’s worth a conversation.

Do I need an assessment first?

For complex journeys, yes. The Assessment & Planning stage includes record review, a needs assessment and a written support plan.

Can you help with ongoing chronic conditions?

Yes. Ongoing journey support can include coordination between specialists, record management, insurance administration and continuity of care.
