The journeydoesn’t endat discharge.

The weeks after hospital are often when patients and families feel most alone. We help organise what comes next, so recovery stays the focus.

Talk About Your Journey
A carer and an older man arriving home with groceries

Does this sound familiar?

Home from hospital, but what now?

Recovery often involves new providers, equipment, therapy and follow-up, all arriving at once.

I just got out of hospital and no one is assisting me with my care.
I need help organising rehabilitation and follow-up.
I don’t know what to do next.

All FAQs
When should I contact you: before or after discharge?

Either. Earlier is helpful because follow-up can be planned in advance, but many clients contact us after they are already home.

Do you provide physiotherapy or nursing?

No. We’re non-clinical. We help source and coordinate rehabilitation, therapy and homecare providers.

How long does recovery support last?

It depends on the journey. Some support is a defined project of 1–4 months. Longer recovery may move to ongoing monthly support.

Can you help with insurance approvals for rehab?

We can help coordinate the administrative side of insurance requests. We can’t guarantee coverage decisions, which rest with your insurer.
