02 · Care Coordination

Many providers. One point of contact.

When care involves several doctors, clinics and services, we keep everything connected: schedules, records and communication, in one organised place.

  • Coordinate appointments across multiple providers
  • Create and maintain healthcare schedules and timelines
  • Organise medical records and documentation
  • Assist with prescription management and coordination
  • Facilitate information sharing between providers, patients and families, where appropriate

03 · Family & Caregiver Support

Support for the people holding everything together.

Caring for someone you love is demanding, especially from a distance. We become the trusted local contact your family can rely on.

  • A single point of contact for family members
  • Updates and coordination when family is overseas or unavailable
  • Help for caregivers navigating healthcare processes
  • Coordination with a designated family contact

04 · Recovery & Continuity

The journey doesn’t end at discharge.

The weeks after hospital are often when families feel most alone. We help organise what comes next, so recovery can stay the focus.

  • Discharge planning support and follow-up coordination
  • Coordinate rehabilitation and home-based services
  • Source homecare providers and community resources
  • Continuity of care after hospitalisation
  • Regular check-ins, as agreed in your package

05 · In-Person Support

Someone beside you on the days that matter.

For key milestones, such as an admission, a planned procedure or an important appointment, we can be there in person, as agreed in your package.

  • Attend appointments, hospital admissions and planned procedures
  • Assist with administrative processes during hospital visits
  • Non-clinical support during key healthcare milestones
  • Basic translation help, or coordination of translation services
